Info Table - PACE

Representatives from San Diego PACE will be here to share information about their comprehensive healthcare program that helps eligible older adults remain safely and independently in their homes for as long as possible. Their model combines both health insurance coverage and healthcare services and is designed for individuals who qualify for Medi-Cal alone or both Medicare and Medi-Cal.

Participants receive all medically necessary care through one coordinated program, including primary care, specialty care, medications, therapy, transportation, and in-home support.
